What to Bring to Your First Visit
To make the most of your appointment, please bring the following. Completing your new patient forms in advance (available for download below) will also save time at check-in.
- A valid photo ID
- Your insurance card(s)
- A list of current medications and dosages
- Any relevant prior medical records or lab work
- A list of symptoms or concerns you want to discuss
- Your pharmacy name and address

Appointment Cancellations
Please notify us at least 24 hours in advance to cancel or reschedule. Same-day cancellations may be subject to a cancellation fee.
Prescription Renewals
Routine prescription renewals require a current appointment on file. Please request refills at least 3 business days in advance.
After-Hours Emergencies
For life-threatening emergencies, call 911. For urgent after-hours concerns, our on-call line is available for established patients.
